When is a health history required?

A health history is required at the beginning of each school year for all students enrolled. Students with no history on file within the first thirty days of school will not be permitted to attend until one is submitted in accordance with Massachusetts General Law 71 section 57. If you are in need of help finding a Provider, please contact the School Nurse.

All new students, as well as Kindergarten, 1st, 4th, and 7th-grade students are required to submit a current physical. The physicals should show the dates of all vaccines received and 7th-grade students should show proof of Tdap vaccine.  For new students, if a current physical is not on hand, the most recent one should be submitted until an appointment can be scheduled for an updated version. For new Pre-K, K, and Preschool students, this physical should include the vision screening before entry into school.

When should I keep my student home?

The school nurse uses the following guidelines when dismissing students, please consider them when deciding if you should send your child to school:

• Temperature over 100 degrees
• Vomiting or diarrhea in the past 24 hours
• Conjunctivitis (pink eye), student may return after 24 hours on medication
• Head lice, may return after adequate lice shampoo treatment
• Pain from earaches, toothaches, headaches and injury that require further evaluation
• Undiagnosed rashes or skin conditions

Casts, Splints, Boots and Crutches: Children with post-injuries or casts, splints, boot or any other medically approved device must provide a doctor’s note clearing them for school. The letter should include information about restrictions if any. This information is needed for us to excuse them from PE and make safe determinations about other activities.

When should I contact the School Nurse?

• How can parent’s best work with the school nurse?
• Whenever a new medication is prescribed for your child.
• In the event of a new medical diagnosis.
• Upon treatment for injury, illness or hospitalization including ER visits.
• To notify regarding orthopedic issues requiring slings, splints or casts.
• To discuss any concern regarding your child’s health and wellbeing.
• In the event your child cannot participate in a class due to illness or injury.

Allergies, Asthma and Medications

Please complete the necessary forms for students with life-threatening allergies, asthma or seizures. Have the Pediatric Provider and a parent or guardian sign them and drop them off with EpiPens, medications and inhalers prior to the first day of school.

Immunization Update

All students are required to have updated immunizations. If you are requesting a medical or religious exemption, those forms are renewed annually as required by MGL c. 76 §§15, 15C, 15D; 105 CMR 220. Please complete these forms before the start of school.
